4th axis

Just wondering if the hat can support a rotary 4th 'A' axis and if so would it work with grbl-mega-5X firmware?

No, the Mega uses another micro controller.

Is there any firmware that does support the 4th axis on the Protoneer hat?

No official firmware that I know of...

I'm not complaining, the board works very well as a 3 axis controller (I have three of them in different configurations). I know the primary target is 3D printers however I'm testing the boards with my mills and lathes. To this end I will be writing a review of my experiences. I realize that functionality is somewhat limited because of GRBL but much can be overcome with post processing, I will be adding a wish list which may or may not be practical.
